Mother's epidural death unlawful

I know theres so much SADNESS on the news lately but this shocked me so much!
What a precious time snatched
Our life together was ripped apart by the action of a midwife who
failed to check the fluid she gave to my wife.
She denied having done so, insisting she
thought it was either saline solution or Gelofusine, a blood volume expander to boost blood pressure.
Thought? I mean HELLO! That should have been checked & KNEW what it was not assumed which is how it looks to me!

SOOOO should this woman go down for manslaughter?
- My answer;
YES .... from what I have read so far deserves to be sent down for manslaughter. The NHS raised their hand immediately after course of death was confirmed. Had she read the label that woman would be living & that poor child wouldn't be left without Mummy!

I know i was gutted to see this on the news! As ive just not long ago had a baby it scared me to death but on the other hand ive worked in the care industry since leaving school i know its easy to make mistakes. But i would want to be punished if i had caused someones death and to leave little ones without a mum!

For once I need to hold back. I know from personal experience (sure you do too), that because of an explosion in childbirth, particularly caused by immigration, that these midwives are run ragged.
Before I get shot down this isn't an anti immigration thing, more like it's a statement against the government. They're coining it from unprecedented immigration without putting proper infrastructure in place before hand. It's like they've opened up the borders to millions, and over time will work to solve the problems created. In the meantime we've all got to endure the growing pains.
The NHS is crying out for midwives.
I don't know enough about the case to know if it was a too-easy mistake to make given the pressure and procedures of the hospital. We all make mistakes, it's just that some people are in postitions when those mistakes can be tragic.
That said, it could well have been pure carelessness. And yeah, agreed, not reading the label is pretty shocking.

I kind of agree with every you said Mr Frugaller but theres a big part of me that doesn't agree that any of the above is part of the life changing mistake this woman made.
I think she was careless made assumtions didn't do her job and there was no excuse. I know when I was in labour the MWs certainly took their time & looked after us nomatter how busy! 30 seconds (if that) of that womans time & this would never have happened.
